Updated Disability Self-Identification FormThe Office of Management Budget (OMB) has renewed the voluntary self-identification form federal contractors must use to solicit disability status information from applicants and employees. The new form was approved without change and is valid until January 31, 2020. Federal contractors are required to either download the renewed form(s) or update their electronic version(s) of the form to reflect the new OMB approval expiration date.

We have learned that Office of Federal Contract Compliance Programs (OFCCP) expects contractors to update any self-identification forms they are using to solicit disability status information from applicants or employees with the new OMB expiration date immediately. Keep in mind the voluntary self–identification form is an OMB–approved form and its content cannot be altered or changed. Therefore, the updated expiration date must be used going forward. Contractors can find a copy of the updated form, including versions in different languages, on OFCCP’website at https://www.dol.gov/ofccp/regs/compliance/sec503/Self_ID_Forms/SelfIDForms.htm.
